The 2025 Hospice Care of Middletown’s
“A Season to Remember” Snowflake Ornament is a lovely way to remember a loved one this holiday season.
This year’s silver & blue snowflake ornament was designed by polymer clay artist & HCoM volunteer, Priscilla Lane.
Each ornament was lovingly hand crafted by a talented team of HCoM volunteers, allowing us to honor our loved ones while supporting the programs and mission of HCoM.
Your loved one’s name will also be displayed on our community Christmas tree located at Woodside Cemetery in Middletown throughout the holidays.
A Hospice Care of Middletown volunteer will call you when your order is ready for pick up.
